| 4 | Shattered Body | Max HP permanently reduced by 10%. |
| 5 | Crushed Leg | Speed reduced by 5 ft. Dashing Speed reduced by 5 ft. as well. |
| 6 | Concussion | Roll Concentration Checks with Disadvantage, or without your Proficiency Bonus. Choose one. |
| 7 | Horrific Scar | You are visibly marked. Gain advantage on Intimidation and disadvantage on Persuasion. |
| 8 | Psychological Trauma | Choose a trigger connected to the near-death event. Exposure requires a Hard WIS Save or become Frightened for up to three rounds, making a new Hard WIS Save at the end of each of your turns. |
| 9 | Lost Fingers | Fine manipulation is harder. Slight of Hand and tool checks at disadvantage |
| 10 | Night Terrors | Roll an Easy Wisdom Save every Long Rest. Failure wakes you up screaming, with one level of Exhaustion. |
| 11 | Crushed Eye Socket | Don’t apply Proficiency Bonus to ranged attacks and Perception checks. |
| 12 | Miraculous Survival | You escape death with only a symbolic scar. You’re convinced it’s divine intervention. Receive Heroic Inspiration if you regale the story of your miraculous survival to a new audience (without them shutting you down). |
